Kabal's face is normal (And he looks and sounds like Elvis, yeah) in Mortal Kombat: Shaolin Monks because the story of that game takes place during Mortal Kombat 2, so this was before he was attacked by Shao Kahn's Extermination Squad led by Motaro in MK 3, so that's why his face is scarred and why he also need a respirator to be kept alive.
